To get started, set up a secure and private account with Fit For Duty K9, fill out all required information, and verify your account. Once your patient (Handler) completes their appointment with your office, sign on to your account, enter the Patient's (Handler's) account number, and follow the prompts to complete and approve the application on the Fit For Duty K9 website.

After completing the application, log out of the Fit For Duty K9 site. Our staff is available for live chat if you have any questions or concerns during the process.
